Job Introduction

Our Urgent Care Response Team provides rapid assessment and treatment to patients in the community, helping to prevent unnecessary hospital admissions and supporting timely discharge from hospital.

As a Band 3 Healthcare Assistant, you will work closely with nurses, therapists and other healthcare professionals to deliver high-quality care to patients in their own homes. You will assist with a range of clinical and personal care tasks, supporting patients with both acute and ongoing healthcare needs.

This is a full-time position working 37.5 hours per week. Shifts are worked between 8:00am and 8:00pm, including weekends and public holidays on a rota basis.

Main Responsibilities

You will be responsible for:

• Supporting registered clinicians to deliver urgent care and treatment in patients’ homes.
• Undertaking delegated clinical tasks such as phlebotomy, catheter care, wound care and pressure area management.
• Monitoring and recording vital signs and reporting any concerns promptly.
• Assisting patients with personal care and mobility where required.
• Maintaining accurate patient records using electronic systems.
• Working independently in the community and as part of a multidisciplinary team.
• Providing compassionate, person-centred care that promotes dignity and independence.

Essential requirements:

• NVQ Level 3 in Health and Social Care (or equivalent experience)
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Enhanced DBS check
• Full, clean UK driving licence
• Access to a car that can be used for work

• Community-based healthcare experience
