**Job Title:** Support Worker
**Reports To: Senior Support Worker / Care Manager
**Location: Sheerness
**Hours: [Full-time / Part-time / Shift-based]
**Job Purpose:
To provide high-quality care and support to individuals, promoting independence, dignity, and choice. The role involves assisting with daily living activities, personal care, and supporting emotional and social wellbeing in line with individual care plans.

Key Responsibilities:
* Support individuals with personal care, such as washing, dressing, toileting, and maintaining hygiene.
* Assist with meal preparation, feeding, and promoting healthy nutrition and hydration.
* Support individuals to participate in social, recreational, and community activities.
* Encourage independence and help individuals achieve personal goals and outcomes.
* Administer medication according to policies and training (where applicable).
* Maintain accurate and up-to-date records of care provided and any changes in condition.
* Follow care plans, risk assessments, and safeguarding procedures.
* Promote equality, dignity, and respect in all interactions.
* Communicate effectively with service users, colleagues, families, and professionals.
* Contribute to a safe, clean, and supportive living environment.
* Report any concerns or incidents promptly to the appropriate person.

Qualifications and Experience:
* Experience in a care or support role (desirable but not essential — training provided).
* NVQ / QCF Level 2 or 3 in Health and Social Care (preferred).
* Up-to-date knowledge of safeguarding, infection control, and health & safety (training can be provided).
* Valid DBS check required.
