PATIENT PATHWAY COORDINATOR

ABOUT ELENIUM HEALTH
Elenium Health is a specialist private practice led by a consultant neuropsychiatrist and Registered Manager, focused on complex neurological rehabilitation, including Functional Neurological Disorder (FND) and Brain Injury. We are entering an exciting period of planned expansion — growing our clinical team, building our MDT, and deepening relationships with referrers — and this role is central to making that happen.
A ROLE THAT GROWS WITH THE PRACTICE
We are honest that Elenium Health is in a growth phase. In the early months, this role will be heavily focused on building — establishing referrer relationships, embedding systems and processes, and creating the operational foundations the practice needs. As the team and patient volume grow, so will the scope and seniority of this position. We are looking for someone who is energised by that prospect, not unsettled by it.
THE ROLE
You will be the operational and relational heart of Elenium Health, working directly alongside the lead consultant. Day to day, you will manage the full patient pathway — from first referral contact through to discharge coordination — while building the client and referrer relationships that will drive the practice’s growth.
You will also maintain the governance and quality infrastructure that underpins safe, CQC-compliant care, supporting audit activity and keeping records and processes in good order.
KEY RESPONSIBILITIES
Relationship management & referrals
– Build and maintain relationships with referring clinicians, solicitors, case managers, and insurers
– Triage and manage incoming referrals
– Proactively follow up with referral sources to maintain warm relationships and support practice growth
– Manage patient and client communications throughout the pathway
– Support onboarding of new patients including consent and administrative paperwork
Patient pathway & clinical operations
– Build and maintain patient appointment timetables, coordinating across clinicians and clinical sites
– Coordinate room and facility bookings
– Coordinate MDT sessions — scheduling, documentation, and minute-taking
– Liaise with associate clinicians to align diaries and workflows
– Maintain accurate records
– Support clinical correspondence and report workflows
Governance, quality & audit
– Support the Registered Manager in maintaining CQC fundamental standards compliance
– Maintain and update SOPs, policies, and procedure documents
– Support internal audits and quality improvement activity
– Manage information governance and consent records in line with GDPR
– Support incident reporting and learning processes
– Help maintain ongoing inspection readiness
Communications & digital presence
– Manage the practice inbox
– Manage professional social media accounts (LinkedIn, Instagram)
ESSENTIAL REQUIREMENTS
– Experience in a private healthcare, NHS, or specialist clinic admin or coordination role
– Working knowledge of CQC fundamental standards
– Experience supporting audits or clinical governance activity
– Outstanding interpersonal skills — builds trust with clinicians, patients, and external stakeholders
– Excellent written communication
– Highly organised with strong attention to detail
– Understanding of information governance and GDPR
– Comfortable with clinical practice management software
DESIRABLE
– Experience in neurological, rehabilitation, or mental health services
– Familiarity with medico-legal referral processes
– Experience coordinating MDT meetings
– Experience managing professional social media accounts
– Relevant qualification in healthcare administration or quality management
