Job Purpose
To provide warm, person centred, one to one support to an individual living with early on set dementia and Type 2 diabetes, who is fully mobile, pleasant, and enjoys conversation and companionship. The role focuses on maintaining the client’s independence, dignity, emotional wellbeing, and daily routines while ensuring safety and comfort within their own home.

Lives with early on set dementia and benefits from gentle reminders and
reassurance.
Has Type 2 diabetes and requires support with healthy meal planning, monitoring,
and prompts.
Is fully mobile and independent with movement around the home.
Has no behaviour issues and is calm, friendly, and cooperative.
Enjoys company, conversation, and social interaction.
Values a consistent, respectful, and patient approach.

Key Responsibilities
1. PersonCentred Daily Support
Provide gentle prompts and orientation to support memory and routine.
Encourage independence wherever possible, offering support only where needed.
Engage the client in meaningful conversation, companionship, and shared activities.
Support with planning the day, maintaining structure, and reducing confusion or
anxiety.
2. Personal Care (as required)
Assist with washing, dressing, grooming, and continence care if needed.
Maintain dignity, privacy, and respect at all times.
Adapt support to the client’s comfort level and preferences.
3. DiabetesAware Meal Preparation & Monitoring
Prepare balanced, diabetesfriendly meals and snacks.
Support with blood sugar awareness.
Encourage hydration and healthy eating habits.
Monitor for signs of hypo/hyperglycaemia and report concerns promptly.
4. DementiaFriendly Support
Use calm, reassuring communication and simple explanations.
Provide memory prompts, reminders, and gentle redirection when needed.
Maintain a safe, predictable environment to reduce confusion.
Encourage activities that stimulate memory, conversation, and wellbeing.
5. Household Tasks
Maintain a clean, tidy, and safe home environment.
Complete light domestic duties such as laundry, dishes, and general housekeeping.
Ensure the home remains comfortable and welcoming for the client.
6. Health, Safety & Record Keeping
Follow all care plans, risk assessments, and Supreme Care policies.
Record daily notes, wellbeing observations, and any changes in condition.
Report concerns promptly to the Registered Manager.
Maintain infection control, food hygiene, and safe working practices.
7. Professional Conduct
Build a positive, trusting relationship with the client and their family.
Maintain confidentiality and uphold the client’s rights at all times.
Work collaboratively with healthcare professionals when required.
Participate in training, supervision, and ongoing development.
Person Specification
Essential
Female (Genuine Occupational Requirement – Equality Act 2010, Schedule 9).
Experience supporting adults with dementia or longterm conditions.
Ability to live in the client’s home for agreed periods.
Warm, patient, and able to hold meaningful conversation.
Respectful, reliable, and able to work independently.
Good written communication for record keeping.
Enhanced DBS check (or willingness to obtain).
Right to work in the UK.
Desirable
NVQ/QCF Level 2 in Health & Social Care.
Experience supporting individuals with Dementia & diabetes.
Dementiaawareness training.
First aid, medication, and moving & handling training.
Full UK driving licence.
Working Pattern
Livein rota (e.g., 1 week on / 1 week off or as agreed).
Daily breaks and rest periods provided in line with Working Time Regulations and
Supreme Care policy.
