Hyde Housing works with housing and supporting vulnerable adults from a variety of backgrounds including mental health, physical health issues, substance misuse, offending backgrounds, domestic violence, care backgrounds etc.
As a housing support worker, you will be responsible for supporting people with their goals, based within their home environments or local community. Duties include supporting vulnerable adults from a variety of backgrounds and personal issues such as mental health conditions, learning disabilities, addictions, homelessness, family breakdowns and criminal histories. Each support worker will have a small number of properties to support.
Activities of support vary from daily living skills mental health, liaising with external agencies such as mental health teams, social services, gps, probation, police, supporting with personal care, teaching of independent life skills to assistance with accessing community-based activities such as going into education, swimming, work placements/experience and other social and leisure activities.
Main Responsibilities include:
– To create and maintain up to date care plans and risk assessments for service users. Monitoring progress and recording notes on daily record sheets relating to each visit.
– To assist Service Users with activities of daily living as per assessment and care plan.
– Review and update care plans on a regular basis.
– Liaise with families, friends, advocates, external agencies and the management team to provide a consistent approach that meets the individual needs of each service user. This could include probation officers, mental health teams, GPs, community liaison officers, social services and social workers, foodbanks.
– To empathise and actively listen in a sensitive manner accessing appropriate additional support where necessary.
– To support Service Users to access appropriate health care provision enabling them to make an informed choice regarding any prescribed treatment, seeking advice where necessary
– Assist with general housekeeping e.g. Cleaning, cooking, shopping, domestic duties etc, and helping Service Users to maintain all aspects of their domestic lives. Supporting and teaching residents independent living skills.
– To create opportunities for Service Users to develop establish and maintain personal relationships and social networks, which encourage greater participation and integration in their chosen communities.
– To assist with accessing community-based activities such as going into education, work placements/experience and other social and leisure activities.
– Liase with professionals to provide client focused and individual support.
Experience:
– NVQ Level 2 or above in Health and Social Care.
– Support Environment, 1 year (Preferred)
– DBS check and clearance.
Location:
Birmingham, West Midlands (Preferred)
Licence:
Driving (Required) with access/use of own car.

Working days/Times:
-Part time hours over 3 days. Ideally Monday, Wednesday, Friday daytime.

WORKING CONDITIONS: Working hours will vary depending on the Service User’s needs and availability of the carer. This can involve either shift work including days, evenings and bank holiday shifts.
