Responsibilities

• Develop and implement personalised care plans tailored to individual client needs
• Act as the primary point of contact for clients, families, and healthcare professionals
• Organise appointments, services, and resources efficiently to ensure seamless care delivery
• Maintain accurate and up-to-date documentation of client interactions and care arrangements
• Provide guidance and support to clients to promote independence and wellbeing
• Liaise with external agencies and service providers to coordinate additional support services
• Monitor and evaluate the effectiveness of care plans, making adjustments as necessary
• Utilise IT systems effectively to record data, schedule appointments, and communicate with stakeholders

Qualifications

• Excellent communication skills in English, both written and verbal
• Proven ability to organise tasks efficiently within a fast-paced environment
• Strong IT skills, including experience with care management software or similar systems
• Previous experience in teaching or training roles is advantageous but not essential
• Compassionate approach with a genuine desire to support others
• Relevant qualifications or training in healthcare, social work, or related fields are desirable but not mandatory
