They will support the day to day running of the Trust’s Tobacco Dependency Service to ensure safe, well organised and evidenced based care is offered routinely to all identified smokers and provided to all that access the service. In line with the NHS long term plan to tackle tobacco dependency, the postholder with support the service and will work with inpatient services to deliver support for any of the BSMHFT service users that require health improvement interventions. S This will include an understanding of working with service users subject to the Mental Health Act and the Mental Capacity Act legislation.
They will provide support in a range of settings as required for the delivery of service. They will organize the workload allocated to them and seek advice when needed from the Public health team manager whilst working under the direct clinical supervision of the Public health lead. They will maintain a high standard of work and prioritise own workload according to the needs of the service, individual needs including equality, diversity, and social inclusion in all aspects of work.
The advisor will work with the team and mental health staff to provide an evidence-based health improvement service by delivering:
o 1:1 and group physical health interventions including tobacco dependency
o Work in a recognised processes to allow staff and service users to access evidence-based physical health interventions and record results.
o The systematic identification of the smoking status of all service users
o The systematic offering of advice, pharmacotherapy, and behavioural support to all current service users
o Working with partners to ensure continuity of support for after discharge.
o Work in a culture whereby all service users are offered the opportunity to stop smoking, engage in physical activity, healthy eating and receive support to achieve a positive health status.
o Maintain their own continuing professional development and be up to date with all required training.
o Liaison with the team manager and project lead to support services users who have recently discharged.
o Ensuring that service users have access to provision of pharmacological support to help them develop healthy behaviours.
o Act as a subject matter resource, advising inpatient services of the most appropriate support they can direct their service users to; based on knowledge of each local community healthy lifestyles service provision.
