Duties

• Assist clients with daily activities such as personal care, mobility, and medication management in accordance with individual care plans
• Support clients in maintaining their independence and promoting wellbeing through tailored activities and social engagement
• Maintain accurate records of care provided, observations, and any changes in client condition using IT systems
• Communicate effectively with clients, families, and healthcare teams to ensure cohesive care delivery
• Follow organisational policies and procedures to uphold safety standards and confidentiality at all times
• Participate in training sessions and team meetings to continually develop skills and knowledge relevant to the role
• Support clients during outings or appointments, ensuring their safety and comfort throughout

Requirements

• Prior experience or background in nursing or healthcare is desirable but not essential; training will be provided if your training is out of date.
• Strong communication skills to engage effectively with clients, colleagues, and external professionals
• Good organisational skills to manage multiple tasks efficiently and accurately record information
• DBS (On update service )
• Basic IT literacy for maintaining electronic care records and documentation
• Ability to work flexibly within organisational hours, demonstrating reliability and professionalism
• Compassionate nature with a genuine desire to support individuals in need
• Ability to work independently as well as part of a team, showing initiative when required
